Display multiple entities within an embedded canvas app?

I am trying to embed the canvas app in D365 (online).  I can see it making all the connections with the relationship within canvas app, but no data is returned within the gallery on the entity form itself when loaded within Dynamics online entity form.  I need to display a list of related data in a table format; like a sub-grid would be used within an entity form.  The data needs to be read only (as I am putting together a "Review" tab screen) so that is why I am trying to use an embedded canvas app.

    I am creating it from within D365 (customize a canvas app option).  It has the connection to the entity item, and it shows the connection with the related entities when I make the gallery's data source the [@ModelDrivenFormIntegration].Item.Documents it does not give me an error.  But when I go back to D365 and pull up the entity form, the canvas app shows nothing in the gallery.

    Are you creating it from within D365 or separately creating and then inserting?
