Hi, I want to have a simple Recorded Service Portal built (a place where outside providers/caregivers can enter things like service provided, start and end time, place of service, caregiver, etc.) but we are on CRM 2013 On Premise. Any thoughts / ideas / suggestions for best methods to do this would be great, including custom web dev portal, adx studio, crm portal (old and new versions).

I recommend contacting a Microsoft Dynamics partner, and getting them to build this for you.
I would suggest, if this is a one time job, then a custom .Net portal will be a good solution. You will have better control over branding ,functionality and UI.
However if you are looking a model driven approach where you want to expose lots of crm entities on portal then something like CRM portal could be a good solution. Having said that CRM portals are available for Online customers.
If the requirement is to replicate the portal functionality like portals/sites for each contact/account whenever they are created in CRM system then SharePoint will be a good idea, as template sites can be easily created in one click, apparently you need to code a bit :)

In theory, the xRM Community Portals Edition might work on Dynamics CRM 2013 (Adx v7 did, even though it wasn't supported). github.com/.../xRM-Portals-Community-Edition I haven't tried it, but it might be worth researching a little bit.
Another option is the AlexaCRM Wordpress plug-in https://alexacrm.com/
There are other 3rd party portal options (Portal Connector) that cost $ but would take less effort than building something from ground up.
